With with the back of the chart number, name and signature.
My question is identical, have you information.

There are two types.
Green strip Issued 1/1/91 CUP 003 Triage 250,000 Landis & Gyr
White strip issued 1/6/92 CUP 003a Triage ??? Landis & Gyr
That' the best I can do other than they where issued to Scottish inmates.

White strip 40 units issued 1/5/95 CUP 005 Triage ??? Landis & Gyr
